Lilly and Walmart Forge Unprecedented Alliance: Zepbound Now Available for Retail Pick-Up at Direct-to-Consumer Prices

Eli Lilly and Walmart have launched a first-of-its-kind partnership, allowing patients to pick up Zepbound, Lilly’s acclaimed weight-loss medication, at Walmart pharmacies nationwide with direct-to-consumer pricing. This collaboration marks a significant shift in medication accessibility, offering unparalleled convenience and financial transparency for millions managing obesity and related health conditions.

In a monumental move set to redefine patient access to critical medication, Eli Lilly and Company and Walmart Inc. have officially announced a strategic collaboration. This partnership introduces a novel retail pick-up option for Zepbound® (tirzepatide) single-dose vials, Lilly’s highly effective weight-loss drug, leveraging Walmart’s extensive network of nearly 4,600 pharmacies across the United States. This initiative is designed to expand access to LillyDirect’s direct-to-consumer (self-pay) pricing, making Zepbound more convenient and affordable for millions of Americans.

The Evolution of Medication Access: LillyDirect Meets Retail

The collaboration, announced on October 29, 2025, represents the first time patients utilizing LillyDirect, Lilly’s direct-to-consumer healthcare platform, can access self-pay pricing for Zepbound vials at a traditional retail pharmacy location. This development is particularly significant given the rapid growth of LillyDirect’s self-pay option, which fulfilled approximately 35% of new Zepbound prescriptions in the second quarter of 2025, as reported by Eli Lilly and Company. This move underscores a broader shift towards more consumer-driven models in healthcare, prioritizing patient convenience and choice.

For patients, this means an additional layer of flexibility. They can now choose between free home delivery from LillyDirect or the ease of local pick-up at their preferred Walmart pharmacy, both offering the same attractive self-pay pricing. This adaptability is crucial for managing chronic conditions like obesity, which often come with significant physical, emotional, and financial burdens.

Strategic Implications and Patient Benefits

The partnership aims to alleviate the multifaceted challenges associated with managing chronic diseases. Jennifer Mazur, SVP and General Manager of LillyDirect, highlighted this, stating, “This collaboration with Walmart is designed to reduce that burden by streamlining access to prescribed treatment. By combining LillyDirect’s innovative, patient-centered platform with Walmart’s nationwide pharmacy footprint, we’re expanding options for patients facing access challenges, making it easier to start and stay on authentic Lilly medicine.”

Walmart’s vast network of nearly 4,600 pharmacies, staffed by over 50,000 trusted pharmacists and pharmacy technicians, plays a pivotal role in this initiative. This extensive footprint ensures that Zepbound becomes more accessible across diverse communities, broadening the reach of this important medication. Kevin Host, Senior Vice President of Pharmacy at Walmart, emphasized the customer-centric nature of the collaboration: “Life is busy, and this will help people discover new, easy ways to get their medication.”

Transparent Pricing for Zepbound Self-Pay Patients

A core benefit of this collaboration is the transparent and competitive direct-to-consumer pricing for Zepbound, making it available without the need for insurance for patients with a valid, on-label prescription. The pricing structure is as follows:

  • $349 per month for the 2.5 mg recommended starting dose.
  • $499 per month for all other approved doses (5 mg, 7.5 mg, 10 mg, 12.5 mg, and 15 mg). Access to the $499 monthly price for doses higher than 5 mg requires patients to meet the requirements of the Zepbound Self-Pay Journey Program, designed to support continuity of care.

These self-pay prices offer a substantial discount—50% or greater—compared to the list price of other incretin (GLP-1) medicines for obesity, a crucial factor in making treatment more affordable and accessible amidst ongoing discussions about drug costs and the availability of compounded alternatives, as noted by Reuters.

Understanding Zepbound: Indications and Safety

Zepbound (tirzepatide) is an injectable prescription medicine indicated for adults with obesity, or some adults with overweight who also have weight-related medical problems, to help them lose excess body weight and keep it off. It is also approved to improve moderate-to-severe obstructive sleep apnea (OSA) in adults with obesity. It must be used in conjunction with a reduced-calorie diet and increased physical activity.

Patients considering Zepbound should be aware of important safety information, including potential warnings and side effects. For instance, Zepbound may cause thyroid tumors, including thyroid cancer, and should not be used by individuals with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). Common side effects include nausea, diarrhea, vomiting, and constipation. A comprehensive discussion with a healthcare provider is essential before starting treatment to review all potential risks and benefits.

The Future of Healthcare Delivery

This partnership between Lilly and Walmart is more than just a logistical convenience; it signifies a pivotal moment in how pharmaceutical companies and retailers are adapting to evolving patient needs. The direct-to-consumer model, empowered by retail giants like Walmart, represents a proactive approach to addressing challenges in medication access and affordability. As Lilly continues to explore additional options to broaden patient access to authentic Lilly medicines, this collaboration sets a precedent for future innovations in healthcare delivery.
